Personal information contained in this form will be used to help us understand where incidents are occurring and help us to reduce hate incidents in Leicestershire.  The information collected will be used for statistical purposes and shared with other organisations that have an interest in reducing hate incidents. Please understand that your personal information collected within this form will not be reported in any instance. If you do not wish to disclose certain information please then tick prefer not to say option .

No one should have to tolerate hate incidents, reporting makes a difference, if we don’t know about it we can’t change things.  This form can be completed by a victim, witness or third person.

What do we mean by a hate incident?

'A hate incident is any incident where you or someone else has been targeted because you or they are believed to be different, this may be motivated by: age disability, gender identity, race, religion / belief or sexual orientation'.
